New clinical handbook launched to support quality abortion care

The World Health Organization (WHO) today released a new handbook for health workers to help them deliver quality abortion services for women and girls. The publication provides detailed clinical advice to support implementation of WHO’s consolidated guidance on abortion care, published in 2022.

Clinical services relating to abortion include not only the procedure itself, but also the provision of information and counselling, pain management and post-abortion care, including contraception. In line with WHO’s updated recommendations, the Clinical practice handbook for quality abortion care additionally provides expanded guidance on how health workers can support self-management approaches, and telemedicine, where this is available.

A few weeks after the supreme court’s 24 June decision to overturn the nationwide abortion rights established by Roe v Wade, the pharmacy chain Walgreens sent Annie England Noblin a message, informing her that her monthly prescription of methotrexate was held up.

Noblin, a 40-year-old college instructor in rural Missouri, never had trouble getting her monthly prescription of methotrexate for her rheumatoid arthritis. So she went to her local Walgreens to figure out why, standing in line with other customers as she waited for an explanation.

Around 73 million induced abortions take place worldwide each year. Six out of 10 (61%) of all unintended pregnancies, and 3 out of 10 (29%) of all pregnancies, end in induced abortion (1).

Comprehensive abortion care is included in the list of essential health care services published by WHO in 2020. Abortion is a simple health care intervention that can be effectively managed by a wide range of health workers using medication or a surgical procedure. In the first 12 weeks of pregnancy, a medical abortion can also be safely self-managed by the pregnant person outside of a health care facility (e.g., at home), in whole or in part. This requires that the woman has access to accurate information, quality medicines and support from a trained health worker (if she needs or wants it during the process).

This week, Georgia became the fifth state to ban abortion at six weeks after a last menstrual period, before many people even realize they are pregnant. Its ban goes further than the others, criminalizing doctors and others who help induce abortions, as well as making those who are pregnant, potentially liable for murder if they prompt a pregnancy loss. They could even be liable if they do it in another state.

On Thursday, Alabama postponed a vote on what could be the country’s most restrictive abortion ban.
