Making Abortion Safe Outside of the Legal System: A Q&A on Self-Managed Abortion

Sociologist Naomi Braine’s new book on the global feminist movement for self-managed abortion took her to Latin America, sub-Saharan Africa, and Europe to study activists’ work there.

From 2017 to 2019, sociologist Naomi Braine, a professor at Brooklyn College, traveled in Latin America, sub-Saharan Africa, and Europe to study what she terms a global feminist movement for self-managed abortion (SMA). The result is her new book, Abortion Beyond the Law: Building a Global Feminist Movement for Self-Managed Abortion (Verso, 2023).

The story of self-managed abortion starts from the fact that, according to the Guttmacher Institute, at least half of all abortions around the world in 2017 were medication abortions, in which people used drugs to end their pregnancies. (The ambiguous legal status of abortion in many countries means that the data is incomplete.) This contrasts with the common image of so-called “procedural” abortion, which occurs under professional medical care and mostly or entirely in a clinic or hospital.

Reproductive health, rights and justice advocates have teamed up to plaster New York City subways with information about how to find abortion pills and access free legal advice about using them. From Queens to the Bronx, in Brooklyn and Manhattan—including Times Square—over 250 posters are now sharing abortion pill information in English and Spanish with the millions of people passing through subway stations and riding the trains each day.

“As politicians and courts continue their assault on abortion access, we are spreading the word that these medically safe and effective pills are available by mail in all 50 states,” said Elisa Wells, co-founder and co-director Plan C, which organized the ad campaign. “Everyone deserves access to this modern medical technology, and we provide the information that people need to take back control of their reproductive autonomy.”

Feminists have been fighting a defensive battle to protect abortion rights for years—but today some are taking the offense, pushing to expand abortion access by calling for the removal of FDA restriction on the abortion pill mifepristone.

Formerly known as RU-486, mifepristone ends pregnancy by blocking the effects of the hormone progesterone, which sustains pregnancy. Used in combination with another drug—misoprostol, which causes contractions to complete an abortion—mifepristone is extremely safe.

Between 1969 and 1973, feminists in Chicago with no formal medical training formed an underground abortion service called Jane that performed nearly 12,000 safe illegal abortions.

Today, as many states increasingly restrict medical professionals’ ability to offer abortion, women are once again finding ways to access safe abortion on their own.
