Mexico’s activist ‘companion networks’ quietly provide abortion pills and support to U.S. women

TIJUANA, Mexico — Just over a decade ago, when Crystal Pérez Lira needed an abortion, she had to leave Mexico. The procedure was illegal in her home state of Baja California and so deeply stigmatized that even Pérez Lira supported the procedure only for those who were raped. Until she unexpectedly got pregnant.

She traveled to the U.S. for help, walking alone across the border from Tijuana to San Diego, first for a health check and a compulsory ultrasound, and then back for a second appointment, when she was given pills to induce an abortion. She returned to Mexico, where she went through the procedure at a friend’s house.

The web is home to an illegal bazaar for abortion pills. The FDA is ill-equipped to stop it.

Experts say the sites pose a public health threat that is likely to grow.

Enter “buy cytotec online cheap” into Google’s search engine and the first four results are sites that illegally offer to ship the abortion pills without a prescription.

Despite the Supreme Court’s ruling last month that gave states the right to ban the procedure, it’s still possible to get abortion medication, even in states where it’s restricted, through telemedicine or by traveling across state lines. But the patchwork of state rules is nonetheless fertile ground for scammers looking to make money off desperate abortion patients who don’t know how to navigate them.

FDA Allows Telemedicine Abortion During Pandemic

Last Monday, the U.S. Food and Drug Administration (FDA) issued long-awaited guidance lifting a restriction on the abortion pill mifepristone for the duration of the COVID-19 public health emergency. The move permits telemedicine abortion, a combination of medication abortion—using pills to end a pregnancy—and telemedicine, which allows health providers to supervise the use of abortion pills via videoconferencing or telephone consultations.

Dr. Janet Woodcock, the acting FDA commissioner, wrote in a letter to the American College of Obstetricians and Gynecologists (ACOG) and the Society for Maternal-Fetal Medicine that the FDA will waive a requirement that clinicians dispense the abortion pill mifepristone to their patients in a clinic or hospital setting. The letter said research studies on telemedicine abortion “do not appear to show increases in serious safety concerns occurring with medical abortion as a result of modifying the in-person dispensing requirement during the COVID-19 pandemic.”

Twenty years ago today, the Food and Drug Administration approved mifepristone, a drug used to terminate early pregnancies that held the promise of revolutionizing abortion care in the U.S.

Colloquially called the abortion pill, mifepristone is taken in combination with another drug, misoprostol, and allows patients under 10 weeks pregnant to have an abortion in the privacy of their home, instead of inside an abortion clinic. Reproductive rights activists lobbying for the drug envisioned a future where women could have the pills prescribed by their primary physician and dispensed at their local pharmacy, transforming abortion into just another part of normal health care.

Feminists have been fighting a defensive battle to protect abortion rights for years—but today some are taking the offense, pushing to expand abortion access by calling for the removal of FDA restriction on the abortion pill mifepristone.

Formerly known as RU-486, mifepristone ends pregnancy by blocking the effects of the hormone progesterone, which sustains pregnancy. Used in combination with another drug—misoprostol, which causes contractions to complete an abortion—mifepristone is extremely safe.

The first wave of prosecutions of abortion pill sellers is upon us. A federal court last week arraigned pill purveyor Ursula Wing on charges of introducing misbranded drugs into interstate commerce and of conspiracy to defraud the United States.

Wing, who lives in New York, is accused of running a website that sold foreign-sourced pharmaceuticals to U.S. customers. The drugs Wing supposedly sold—mifepristone and misoprostol—can be taken in a two-step process to induce an abortion. The U.S. Food and Drug Administration has approved this pill regimen for prescription use, under the brand name Mifeprex.
