Three Years Post-Dobbs, Abortion Providers Experience High Levels of Violence & Disruption

NATIONAL—Today, the National Abortion Federation (NAF) released its Violence & Disruption report with data from 2023 and 2024. The report shows there has been sustained and consistent harassment and violence against abortion providers, even as clinics closed and abortion became harder to access in some regions.

This year’s interactive report compares the violence and disruption that NAF members reported in 2023 & 2024 to the total data NAF has tracked since 1977. The report features “heat maps” that demonstrate the states where providers reported experiencing the highest levels of obstruction, protesters, threats, and trespassing. Finally, the report includes an audio storyteller map where viewers can click through and hear directly from providers at clinics across the country about their experiences with violence and disruption.

Abortion funds and logistical support groups are enabling people to travel out of state to obtain abortion care.

After the Supreme Court’s June 2022 Dobbs decision eviscerated the already limited federal right to abortion, 14 states — Alabama, Arkansas, Idaho, Indiana, Kentucky, Louisiana, Mississippi, Missouri, North Dakota, Oklahoma, South Dakota, Tennessee, Texas and West Virginia — banned the procedure.

In some of these states, clinics closed. According to The Guardian, 42 U.S. clinics shuttered in 2022, plus 23 more in 2023. But as disturbing as this is, it is not the full story. Despite financial, legal and political obstacles, many clinics in states that have banned abortion have pivoted, continuing to provide essential reproductive health services such as contraceptives, STI testing and treatment, and routine gynecological exams, with some expanding to deliver prenatal and gender-affirming care. In addition, new clinics have opened in states like Wyoming and Maryland where abortion remains legal.

As South bans abortion, thousands turn to Illinois clinics

FAIRVIEW HEIGHTS, Ill. (AP) — Dr. Colleen McNicholas is fresh off performing two abortions when a ringing phone quickly stops her.

“Oh, ugh,” she said, eyes widened, before she darted off to another room.

Just the day before, 58 women had abortions at the Fairview Heights’ Planned Parenthood clinic, 15 miles east of St. Louis. But the new day is still stacked with appointments; as many as 100 abortion and family planning patients might walk through the doors.

There were once 30 clinics performing abortions in Missouri. Now there’s just one. It’s on the front line of a battle to defend a woman’s right to choose.

Colleen McNicholas is always watching her back. The location of her family home is protected. The perimeter is guarded with security cameras and alarm systems. Her child’s identity is kept secret. As she drives to work, she checks her rear-view mirror to make sure no-one is following. She never takes the same route two days in a row.

It’s not the typical routine for a doctor travelling to their surgery. But in the conservative mid-west state of Missouri, Dr McNicholas’s line of work makes her and her family a target for extremism. She’s a doctor at the state’s last abortion clinic, the only centre still providing safe and legal abortions for a population of 1.1 million women. If the powerful anti-abortion lobby gets its way, it too could soon be shut down.

Almost four years ago, I sat on a cable-news set waiting for the Supreme Court to hand down a ruling on a Texas abortion law that, reputable medical organizations agreed, amounted to a bogus justification for shutting down abortion clinics. The live feed was trained on candidate Hillary Clinton’s Cincinnati rally, featuring Elizabeth Warren, who had just endorsed her.

Alike in blonde bobs and jewel tones, if not much else, the two raised their clasped hands to the sky in a show of party unity and the hint of an all-female ticket, or at least a future in which reproductive autonomy, along with everything else, didn’t depend on the whims of a tiny number of white men. The particular man we were waiting on that day was Justice Anthony Kennedy. Minutes later, the networks cut away to announce that his vote in Whole Woman’s Health v. Hellerstedt would keep the clinics open, ruling that the Texas law placed an unconstitutional burden on women.

Laurent Delli-Bovi is used to operating her Brookline, Massachusetts, abortion clinic in a state of financial precarity. Women's Health Services, which has been around for almost 28 years, has been in the red for the last 13 of them.

Delli-Bovi, the clinic's medical director, said those years have mostly consisted of "robbing Peter to pay Paul": putting off paying some bills in favor of more urgent ones. The independent clinic runs on a "day-to-day" basis, its future never guaranteed.

According to the Guttmacher Institute, roughly 75 percent of those who get abortions are poor or low-income — not necessarily a surprise, given the lack of access to affordable preventative health care and contraception. Unlike most medical procedures, the majority of states don’t cover terminating a pregnancy through Medicaid (with very narrow exceptions), leaving patients to pay for the procedure out of pocket. But for low-income patients — especially in rural areas across the country — finding the funds to pay for an abortion out of pocket is quite literally only half the battle.

The other half? Paying to get to the procedure itself — a task that can cost hundreds of dollars on its own and eat up hours, if not days, of travel time in states that lack usable local public transit systems or mass transportation between rural and urban areas.
