Four years after the Supreme Court handed states the power to ban abortion, a striking paradox has emerged: Abortion rates are rising.

Since Roe v. Wade was overturned in 2022, the number of abortions in the U.S. has increased over 20 percent, according to the Society of Family Planning’s latest #WeCount report, released June 10.

Since Roe fell, a community-led network of care has grown into a nationwide system with the promise of “mife no matter what.”

In June 2022, after the Supreme Court overturned Roe v. Wade and over half of states banned or restricted abortion, grassroots activists across the country organized mutual aid groups to share free abortion pills with people living in restrictive states. Today, community providers distributing free abortion pills operate in every U.S. state and territory that bans or restricts abortion.

Eighteen months after the Dobbs v. Jackson decision that overturned the constitutional right to abortion, and with a new Supreme Court challenge pending against the abortion medication mifepristone, confusion abounds about access to reproductive health care in America.

Since the June 2022 decision, abortion rates in states with restrictions have plummeted, and researchers estimated last month that the Dobbs decision led to “approximately 32,000 additional annual births resulting from bans.” Journalists profiled women who carried to term since Dobbs because they couldn’t afford to travel out of their restrictive state.

Over half of clinician-supervised abortions in the U.S. in 2020 were done with a combination of two medications: mifepristone and misoprostol. A Trump-appointed judge in Texas will soon decide a lawsuit brought by anti-abortion extremists asking him to force mifepristone off the market in all 50 states. If he does, as anticipated, reproductive rights advocates are ready to offer a safe and effective alternative to end pregnancy through three months: a higher dosage of misoprostol taken alone.

Misoprostol is a widely available ulcer medication that can induce a miscarriage by causing contractions of the uterus to expel a pregnancy. In the 1980s, Brazilian women began using misoprostol to end their pregnancies because abortion was unavailable through the medical system. Self-managed abortion with misoprostol resulted in precipitous declines in infection, hemorrhaging and death from unsafe abortion.
