The Trump Administration is expanding foreign aid restrictions on abortion, transgender health care, and DEI initiatives.

On January 23, Vice President J.D. Vance launched the Trump Administration’s new plan to “promote families and human flourishing.” But rather than being, as he claimed, “pro-life,” these rules will threaten the lives of people around the world, especially women and people who don’t fit into the administration’s narrow, unscientific categories of gender.

The first of the new restrictions on foreign aid announced by Vance extends the existing Mexico City Policy prohibitions on abortion funding to encompass not just global health assistance but all non-military foreign assistance—including U.S.-based nonprofits and government-to-government funding. Known to critics as the “Global Gag Rule,” recipients receiving funding from the U.S. government are prohibited even from tapping other donors to provide information or education regarding women’s health. This expansion will severely limit access to abortion and the full range of sexual and reproductive health care, even in humanitarian emergencies.

“The mark of barbarism is that we treat babies like inconveniences to be discarded,” Vice President JD Vance bellowed to a crowd of zoomer nuns, bagpipers, and white nationalists at the annual March for Life in Washington, DC, last Friday.

The vice president then proceeded to announce a threefold expansion of the Mexico City policy, a decades-old, controversial foreign policy that prohibits organizations from receiving foreign aid if they mention abortion as a family planning option. It was reinstated last year when President Donald Trump resumed office.

WASHINGTON (AP) — The Trump administration is expanding its ban on U.S. foreign aid for groups supporting abortion services to include assistance going to international and domestic organizations and agencies that promote gender identity as well as diversity, equity and inclusion programs.

An administration official said Thursday that the State Department would release final rules that expand the scope of the “Mexico City” policy that has already severely reduced assistance to international organizations that provide abortion-related care. The policy was first established under President Ronald Reagan, rescinded by subsequent Democratic administrations and reinstated in Trump’s first term.

After a complete freeze on foreign aid spending when Trump took office, the US is now in the process of striking new funding agreements with African governments. These promise aid money in exchange for certain conditions – from mining rights and access to valuable patient data, to agreements to spend national health budgets on priorities dictated by America. The deals replace a patchwork of previous health agreements under the United States Agency for International Development (USAID), which has been dismantled during Trump’s first year back in the White House.

Holding the Trump Administration Accountable for Wasting Millions of Dollars of Birth Control

The Trump administration will let tens of millions of dollars’ worth of contraception paid for by taxpayers expire rather than distribute it as foreign aid. The contraception, representing a value of at least $10 million and possibly as much as $40 million, was earmarked for women and girls in sub-Saharan Africa—where its loss could result in more than a million unintended pregnancies and thousands of maternal deaths. Multiple humanitarian organizations have offered to buy the birth control, but the administration has refused.

On August 29, 2025, the Center for Reproductive Rights filed a Freedom of Information Act (FOIA) request for information about the administration’s decision to let the contraception—and millions in taxpayer money—go to waste. The administration has not responded to the request. The Center is now suing.

The Trump administration has decided to destroy $9.7m worth of contraceptives rather than send them abroad to women in need.

A state department spokesperson confirmed that the decision had been made – a move that will cost US taxpayers $167,000. The contraceptives are primarily long-acting, such as IUDs and birth control implants, and were almost certainly intended for women in Africa, according to two senior congressional aides, one of whom visited a warehouse in Belgium that housed the contraceptives. It is not clear to the aides whether the destruction has already been carried out, but said they had been told that it was set to occur by the end of July.

Since taking office, President Donald Trump has implemented an unprecedented cutoff of U.S. foreign aid. On 20 January 2025, he froze international assistance for 90 days, claiming that this was so the federal government could review and ascertain whether U.S.-supported programs reflect U.S. interests and values. On 24 January, the State Department issued a “stop work” order, pausing all existing and new foreign aid. Secretary of State Marco Rubio initially provided for some exceptions to that order, including food assistance and military support for Egypt and Israel. On 29 January, he issued an additional waiver so that “life-saving humanitarian assistance” including medicine and other supplies would continue to flow, although it is unclear whether this has happened in practice. In early February, in an alleged effort to reduce federal spending, the administration subsumed the U.S. Agency for International Development into the State Department. Toward the end of the month, a flurry of contract cancellations and litigation added further to enormous global uncertainty about the future of U.S. foreign assistance – and led to disruptions in services ranging from famine relief to HIV treatment.

Shortly after taking office, the Trump administration issued a cascade of executive orders—among them, the reinstatement of the Mexico City Policy, commonly referred to as the global gag rule (GGR). The GGR restricts U.S. funding for international nongovernmental organizations (NGOs) that provide, refer for, lobby or counsel on abortion care. While much has been covered about the GGR’s adverse effects on global health and NGOs, its impact on vulnerable populations in humanitarian settings, particularly refugee women and girls, has received less attention.

In 2024, a record 122.6 million people worldwide were forcibly displaced due to climate change, political unrest, persecution and armed conflict. More than 90 million women and girls are expected to require humanitarian aid, including sexual and reproductive health services, such as perinatal and abortion care, gender-based violence (GBV) support, and STI screening and treatment.

In Nepal, aid cuts and Trump presidency threaten safe abortion gains

Gains made in Nepal on lowering maternal deaths from unsafe abortion stand to be undone by a drastic drop in interest from international donors, experts have warned — and a Donald Trump return to the White House could make things worse.

Nepal’s remote and mountainous topography remains a barrier to accessing health care, but huge strides have been made in recent years toward lowering maternal deaths from unsafe abortion. After legalizing abortion in 2002, the maternal mortality ratio dropped from 539 maternal deaths per 100,000 live births in 1996 to 151 in 2021, and the severity of complications from unsafe abortions significantly reduced during that period.

Project 2025 Is Already Here

Core aspects of the far-right plan to overhaul U.S. government are already being put into place, through an anti-abortion influence campaign overseas.

When pundits, critics and supporters discuss Project 2025, the Heritage Foundation’s readiness work plan for a second Donald Trump presidency, it’s always in the future tense. At more than 900 pages, Project 2025’s playbook, Mandate for Leadership: The Conservative Promise, is a door-stopper of policy recommendations that lays out detailed steps for decimating democracy in the first 180 days of the new administration. Some executive orders include eliminating the Department of Education (“a woke education cartel”), renaming the Department of Health and Human Services the “Department of Life,” and anchoring these commitments in the promise to restore “the family as the centerpiece of American life.”

This Christian nationalist plan is no fever dream: even if Trump loses in November, many core aspects of Project 2025 will still be implemented. In fact, some of its recommendations are already underway.
