A small but influential number of medical practitioners who obstruct abortion care or are uninterested in providing it are leaving women unable to access abortion in many parts of rural New South Wales, a study has found.

It has led to informal and often underground networks of health workers providing information and access to abortion care to patients, the study found, with these providers burning out due to high demand and attitudes toward them.

A common, safe, and effective abortion pill could be yanked from the market or placed under restrictions by midnight Friday, depending on the outcome of the first major abortion case to hit the Supreme Court since Roe v. Wade was overturned last year. Abortion clinics across the country have spent weeks bracing for this moment, consulting with lawyers and rapidly recalculating if and how they will perform abortions if they’re forced to change how they use the drug, mifepristone.

But regardless of any ruling from the Supreme Court on the fate of mifepristone, the nation’s highest court can only control the legal market for the drug. It has no real ability to dictate what happens within the thriving world of underground abortion networks—where mifepristone has continued to flow and where demand for the drug will likely skyrocket, rather than fall, if the Supreme Court tries to cut off the U.S. health care system’s supply.

RIGA, Latvia — For two sunny, crisp autumn days in mid-September, Riga’s Stradiņš University felt like the epicenter of a self-styled global civil rights movement: to give every person, in every culture or country, regardless of laws, access to abortion pills.

In the hallways, women pored over posters showing the latest research on the effectiveness of abortion pills and other developments in abortion and contraception care. Representatives from pharmaceutical companies enthusiastically pitched their medications and products to doctors sipping coffee and tea during a break between panels. There were graphic novels about an at-home medical abortion and T-shirts printed with women’s self-stated reasons for ending a pregnancy; there were slogans printed on T-shirts like “Make Abortion Legal Again” and a video promoting abortion rights to the tune of Lady Gaga’s “Bad Romance.”

On a winter morning, Anna* walked the aisles of an herbal-medicine store, picked up a bottle each of blue cohosh and black cohosh, along with a plastic bag of pennyroyal tea, and drove to the topless bar on the edge of town where she worked. There, she met Jules, another dancer. They performed on a small stage with crystal curtains, the green light of an ATM flashing on their left, until 9 p.m. The women, both in their 20s, then drove to the Motel 6 where Jules lived and entered her dim room on the second floor, which smelled of grape cigars. Anna pulled out the tinctures and tea and explained the plan. She was going to help Jules try to have an abortion.
