Sexual and Reproductive Health Is Fundamental to Achieving Universal Health Coverage

World leaders will gather in New York City on Sept. 21 for the 78th session of the United Nations General Assembly (UNGA) to review progress toward universal health coverage (UHC). They will face this critical moment amid a hostile political climate—sexual and reproductive health (SRH) is being politicized and threatened (both in the U.S. and globally). Powerful anti-rights groups are lobbying to restrict access to critical SRH services and to exclude safe abortion care from health and development initiatives. How can advocates and activists harness this global convening to combat the anti-choice movement while protecting and expanding SRH, including access to safe abortion?

The goal of UHC is to create just and effective health systems that deliver healthcare services that cover everyone, everywhere, regardless of their ability to pay. To be truly just and effective, any UHC package must include comprehensive SRH services so that all people can fully protect their health and exercise their right to bodily autonomy and reproductive self-determination. When access to abortion is restricted, many people are left unable “to determine whether or when they have children and may be forced to continue unwanted pregnancies or end pregnancies in dangerous and degrading ways.”

As technology advances, healthcare laws, particularly concerning women are becoming regressive world over. And way things are moving, with increased restrictions to women accessing a basic right to health like that of access to safe abortions is becoming a Herculean task.

It is time that abortions are treated as a critical healthcare service which should be made available to every woman in a timely fashion and in a dignified, non-judgmental environment.

Access to abortion is an unconditional human right and should be entitled as a basic health care service delivered as a part of Universal Health Coverage (UHC). UHC is a commitment to equity and focuses on ensuring a level of quality care that helps people achieve their desired health outcomes.

While on the surface it looks easy,
pro-choice, abortion, and autonomy over women’s body face barriers in the
generational prejudice against women, existing taboos, and pedagogical approach
towards a division of labour. These various components of disparities have led
to the gross neglect of women’s health, and the most overlooked is the Sexual
and Reproductive Health Right (SRHR), which is essential to the fulfillment of
universal health coverage and is included in the Sustainable Development Goals,
which commit countries to providing universal access to sexual and reproductive
health services by 2030.

Abortion is health care, and health care is a human right. That’s why efforts to advance universal health coverage (UHC)—an international effort to guarantee that all people, regardless of where they live, have access to essential, quality health services without financial hardship—must include strong language defining sexual and reproductive health care, including abortion care, as an integral part of health and well-being. Ipas is committed to working with the World Health Organization, governments and other partners to attain the Sustainable Development Goal targets, which include achieving UHC.

“The world still has far to go to achieve gender equality,” said Ipas Senior Technical Manager for Community Engagement Tanvi Monga in a recent opinion for Global Health Now. “Women shoulder the burden of child care, elder care, household care, family health and health-care costs—and for poor or near-poor women anywhere in the world, health-care costs can cause irrevocable financial strain.” Plus, health-care services labeled as “for women” are frequently separated from other services—and are harder to access or more expensive.

What was it about: some history

This conference has taken place every five years, beginning in 1994. At each follow-up meeting, the overarching purpose has been to measure progress (and the lack of progress) in implementing the 1994 Programme of Action, which was agreed by acclamation by the representatives of 179 countries, and the follow-up actions added at subsequent conferences. An excellent summary of the aims, goals and history of the conference can be found here and a 20th anniversary edition of the Programme of Action can be found here along with a global report on progress published in 2014.

In 1994, UNFPA, the conference convenor, described the Programme of Action as: “a bold new vision about the relationships between population, development and individual well-being… remarkable in its recognition that [sexual and] reproductive health and reproductive rights, as well as women's empowerment and gender equality, are cornerstones of population and development programmes. The Consensus is rooted in principles of human rights and respect for national sovereignty and various religious and cultural backgrounds.
