Some of Onikepe Owolabi's most vivid memories of medical school in her native Nigeria are of the teenage girls she saw in the emergency room of a rural hospital with complications from an unsafe abortion — painful infections that, if left untreated, can lead to permanent disability or even death.

Each time, Owolabi, now a senior research scientist with the Guttmacher Institute, a nonprofit reproductive rights organization in the U.S. that supports abortion rights, assisted doctors in promptly providing the girls with a group of essential obstetric services known collectively as "post-abortion care," or PAC.

US President Joe Biden has reversed a Trump administration policy that prohibited US funding for nongovernmental groups that provide or refer patients for abortions.

The Global Gag Rule, also known as the Mexico City Policy, was enacted in 1984 by President Ronald Reagan. Since its introduction, the policy has been instated by each Republican president and rescinded by each Democrat president.

On Thursday, Joe Biden is expected to participate in what’s become a regular post-inauguration ritual for U.S. presidents by signing an executive order repealing the Mexico City Policy, known by its opponents as the global gag rule, which prohibits U.S. funding to foreign nongovernmental organizations that provide abortion counseling or referrals. The policy is a textbook case of how the lives of people thousands of miles away can be directly affected by America’s culture wars. And while Biden’s move will be applauded by reproductive rights advocates and family planning service providers around the world, the uncertainty and instability the rule introduced will be hard to erase.

A Biden Promise About Overseas Abortion Funding Could Prove Difficult To Implement

Among the promises that President elect-Biden is expected to fulfill immediately upon taking office: lifting a ban that President Trump imposed on U.S. foreign aid dollars related to abortion.

Specifically, the ban prohibits foreign aid funding for privately-run overseas-based groups that "perform or actively promote abortion as a method of family planning" for women. Among the activities that are barred: providing referrals for abortion or offering advice or medical information on the procedure. And this is prohibited even if the group funds their communications around abortions with non-U.S. sources.

On a rainy morning in May 2019, Dr. John Nyamu was attending to patients on the cluttered first floor of an office building in downtown Nairobi when he heard raucous shouts from down the street. A caravan of protesters was winding toward him, a few hundred people teeming in the streets, bellowing through loudspeakers, and stopping traffic.

As the crowd reached his building, Nyamu, a well-known gynecologist who performs abortions in a private clinic, peered through his window at the protesters below to make out what they were saying. It turns out they were targeting him. “Abortion is murder! Abortion must go! Nyamu must go!” Some held signs with photos of mutilated fetuses. Others clutched baby-size cardboard coffins with crosses on them.

The Trump administration’s war on abortion rights is worse than you think

The Trump administration has been executing a coordinated attack on what it sees as a critical public health issue. Unfortunately, the offensive is not targeting the COVID-19 pandemic, which has infected over six million people and claimed almost 200,000 lives in the US. Instead, the campaign has its sights set on women’s sexual health and reproductive rights, especially abortion. With the recent death of Supreme Court, Justice Ruth Bader Ginsburg threatening the fate of Roe v. Wade, the security of abortion rights has never been more precarious.

The administration’s brazen anti-abortion agenda includes not only well-publicized executive actions such as the expansions of the global and domestic gag rules, “conscience” exemptions to the Affordable Care Act’s contraceptive mandate, and the packing of courts with anti-abortion judges.
